*Qualifications: High school diploma or GED preferred but not required for position.
Must have evidence of the ability to work constructively with developmentally disabled individuals.
Previous direct care, nursing and / or developmental disabilities experience preferred.
Must demonstrate basic* *respect and good will toward individuals and co-workers.
Must possess good general health and be able to lift or help lift adults.
Must have a valid driver's license.
May be required to transport individuals for activities doctor's appointments, etc.

Basic Function: To possess and practice a commitment to provide a home environment to the individuals residing in the home.
The Direct Care Professional will work directly with the individuals both individually and in groups the help meet their various social, physical and psychological needs.
Primary Responsibilities:
* Provide direct care and habilitation programming to individuals.
* Maintain adequate personal hygiene of individuals by assisting with bathing, grooming, dressing and undressing, feeding, toileting and positioning.
* Promptly report to the nurse, Shift Manager, or Facility Manager any suspected need for medical attention.
* Assist with or perform nursing care procedures or routines as requested.
* Serve as chief resource person for the day-to-day status and needs of the individuals.
* Work with the professional staff to evaluate assigned individuals, present abilities, progress and potential.
* Participate in the development and follow- through of assigned individuals IP's.
* Follow and document the program and activities schedule on each individuals.
* Properly use and care for each individual's adaptive and personal equipment: i.
e.
, wheelchair, eating utensils, T.
V.
* Interact with each individual and co-worker in a manner which is consistent with the utmost respect for each person.
Call each individual only by their name- no derogatory or made-up nicknames are allowed to be used.
* Interact physically with each individual and co-worker in a socially and professionally acceptable manner.
* Use acceptable language and tones of voice.
* Maintain a safe, clean, healthful environment for individuals and co-workers, including laundry and sanitizing dining room tables before all meals and snacks.
* Exchange pertinent information about individuals and other concerns with the Shift Managers, the Facility Manager, the nurse and other personnel at the beginning and close of the shift.
* Keep Shift Managers or Facility Manager informed of any changes in individual or work schedule.
* Participate in on-going staff development activities.
* Follow Horizons, Inc policies and procedures in compliance with HIPAA Privacy Rule.
* Other related duties.
