The Tuscarawas County Board of Developmental Disabilities (TuscBDD) arose from a grassroots effort by parents and concerned citizens in 1955 to provide educational services to 12 children with developmental disabilities through the creation of Starlight School.
Over time, Starlight School has evolved into TuscBDD, which is a publicly funded county governmental entity offering an array of programs for over 500 eligible individuals.
We officially became Tuscarawas County Board of Developmental Disabilities in October 2009.
We are here to ensure that these residents have available to them the programs, services and supports necessary to help them enjoy a fuller, richer life.
Regardless of our size, name, and funding sources, our product has remained the same ~ quality services to eligible individuals for all life stages: * Early Intervention (Children) * Preschool (Children) * School Age (Children) * Vocational Habilitation (Adult) * Day Habilitation (Adult) * Supported Employment (Adult) * Activities (Adult) * Transportation * Service & Support Administration (SSA) * Family Support * Residential Under direct supervision of the Service and Support Administration Director and Manager, an SSA at TuscBDD provides a primary point of coordination in the intake, assessment, plan and budget development, service coordination and monitoring, and information and referral services to and on behalf of individuals receiving service and support services; conducts eligibility determinations; assures the health, safety, and welfare of individuals receiving service and support services; consults Board staff, residential staff, parents, and others in the individual’s circle of support in the development of specific goals and objectives on the Individual Support Plans (ISPs) of persons receiving service and support services in accordance with their desired services and supports; provides initial ISP packet development, program review and approval, quality assurance/monitoring, annual re-determination, ISP revisions, provider recruitment, training and support, waiting list, utilization review and other waiver activities to and on behalf of individuals receiving service and support services.
*Qualifications:* * Bachelor’s Degree in Education, Psychology, Social Work, or related field * Must acquire and maintain Service and Support Administrator Certification through TuscBDD * Must be willing to work a flexible schedule and assume on-call status on a rotating basis with other personnel * Must hold and maintain a current, valid driver’s license * Must maintain insurability with automobile insurance carriers * Must acquire and maintain Board approved first aid and CPR certification * Must have keyboarding experience and experience with word processor, database, and spreadsheet applications *ESSENTIAL DUTIES:* * Conducts evaluations and assessments and develops initial Individual Service Plans (ISPs) using appropriate and approved discovery tools as determined by the agency Administration, including but not limited to habilitative evaluations, behavioral assessments, waiver Levels of Care, and individual waiver and/or supported living determinations and re-determinations; coordinates outside services and/or evaluations.
* Plans and conducts meetings to develop, revise and review ISPs of individuals receiving service and support services, coordinating with personnel from ICF/IIDs and/or waiver homes, other provider agency representatives and others from the individual’s circle of support as necessary and appropriate; facilitates the teaming process with appropriate members of the individual’s circle of support; composes ISPs of persons receiving service and support services to provide services and supports in accordance with the individual’s wants and desires and available resources.
* Assists people supported and their families as appropriate, with obtaining education, accessing community medical services, Medicaid State Plan services, other resources, and benefits in order to meet the needs of the person.
* Maintains the health, safety, and welfare of individuals receiving services and supports through monitoring and tracking incident reports and by immediate reporting of suspected abuse, neglect, exploitation, or misappropriation of funds; Provides crisis/emergency intervention by assuming on-call responsibilities in rotation with other Service and Support staff; assures the confidentiality of all information related to individuals receiving service and support services.
* Assists individuals receiving service and support services and their significant others in identifying, prioritizing and designing desired services and supports and in accessing the resources necessary to meet their needs and foster desired outcomes, including assistance in the development of individual budgets.
* Maintains a current file of all individuals on the Service and Support Administrator’s caseload, including all documentation required by law, rule or policy, and including monitoring and documentation to assure that services and supports identified on the ISPs of persons receiving service and support services are being delivered in an appropriate and timely manner.
* Researches and maintains a list of providers of services and other resources, assists individuals and families in choosing and accessing these service providers and resources, and acts as a liaison between individuals and their families and these service providers and resources; ensures that the individual’s services are effectively coordinated and provided by appropriate providers.
* Assists with Individual Information Form reporting process through the completion of individual change forms and verification of needed information.
* Conducts eligibility determinations as assigned.
* Completes related forms and reports as is necessary.
* Coordinates the development, staff training, and implementation of behavior support plans as is necessary.
* Participates in meetings, committees and inservices as required.
* Manages time and multiple tasks effectively and independently.
* Assists in public relations activities.
* Requests and/or purchases supplies as needed.
* Demonstrates regular and predictable attendance.
* Follows applicable laws, rules, regulations, standards, and Board policies and procedures.
* Carries out related duties as requested.
*EXPECTATIONS:* 70% Productivity (Targeted Case Management) 50% Time in Field with Individuals Job Type: Full-time Pay: $40,000.
